AUTHORIZATION & CONSENT FOR SHARED INFORMATION

I authorize and give full permission to 323 Logistics LLC (the “Company”) to share with

Amazon Logistics, Inc. (“Amazon”) the results of drug and alcohol tests, compensation

information, performance review or documents, information regarding disciplinary

actions, personally identifiable payroll data, financial account numbers, and any other

information that Amazon may request from the Company in connection with Amazon’s

right to audit Company records.

Any confidential and/or private information about you provided to Amazon will be kept

confidential by Amazon, and access to your information will be limited to those with a

business need to know.

I acknowledge that my signing of this authorization and consent form is a voluntary act

on my part and that I have not been coerced into signing this document by anyone.

I accept the terms of the agreement outlined above.
